Model – Volunteer Agreement

Conference Registration:

All fashion show models are required to fully register as an attendee of the PCOSA Conference being held at The Hilton Rye Town from October 7-10, 2004

Release Form:

All fashion show models are requires to sign and return the attached model release form

Required Volunteer Service:

As a fashion show model you are required to participate in orientation and dress rehearsal Friday, October 8, 2004 , location to be announced, from 6 pm – 10 pm and dress rehearsal, styling and orientation on Saturday October 9, 2004 in the assigned dressing rooms starting at 5:30PM

Perks:

In return for hours worked and your conference registration, you will be entitled to a model bag filled with special gifts from our fashion show partners. You will receive free services for make-up, hair and nail care on the assigned fashion show dates. You will be provided with a meal prior to the fashion show and your registration for the gala and fashion show will be waived.

Please return the following volunteer information expressing you have read and agree to all terms mentioned above.

I have read the requirements for volunteering for The PCOSA 2004 Fashion Show and agree to the following terms:

• I agree to register for the conference and return all requested forms to participate as a model
• I agree to show up for all of my shifts or will give ample notice if I will be late or unable to perform my responsibilities
• I will notify the fashion show coordinators if there are any changes in my clothing sizes between now and the conference

I understand that failure to follow the mandate set by PCOSA will result in one or all of the following:

• Will have to pay the $50.00 fee to attend the fashion show and gala.
• Loss of the privilege to model for PCOSA Fashion Show in the future
